Where Do I Get Started?
We know that it can be stressful to navigate the insurance coverage process, but we strive to make it as smooth as possible. We accept a variety of plans from insurance companies at all of our locations. To verify if your insurance plan is accepted for services in our office, please review our list of accepted insurance companies below. Have further questions? Contact your insurance company to go over an explanation of benefits.
If your insurance company is accepted: Please contact your insurance company to verify what care is covered by your plan as well as information on copayments, deductibles and coinsurance amounts. Our office will pull an explanation of benefits before your appointment with the information provided. This information does not guarantee patient eligibility, patient coverage, or payment to providers. Confirmation of eligibility will be determined upon receipt of any claims.

If your insurance company is not accepted: This means your insurance plan is considered out-of-network which can lead to greater out-of-pocket expenses for your care. To combat that, Curtis Eye Care can provide an out of pocket discount off services.

*Out of pocket visits are offered at a discounted price. The base price they start at varies by the type of appointment and any additional testing required.
For an Eye Exam with Glasses Prescription: $75.00
↪ Adding on a Contact Lens Prescription: $70.00 to $110.00
For a Medical Exam: Starting at $120.00
↪ This includes eye health issues or emergencies.
